Print and complete this form if you have critical medical equipment that requires electricity to operate. The completed, physician-signed form will allow your name to be placed on a power-restoration high priority list. During an outage, priority will be given to restoring power to accounts with known medical needs. It is our recommendation that households with medical equipment have emergency plans in place for power outages (small generators, battery-operated alternatives, etc.).
